3 years ago last September I had a mitral valve repair, after the operation was 99.9% fitter than I had been in a long time (a big thank you to Southampton hospital) two-two and half years later symptoms returned, was eventually refered back to the cardiolagist they confirmed that it had a slight leak and would monitor it. However Christmas I came over bad on the way to work, suspected HA, this was dismissed after the initial blood tests and refered back to cardiology, they think that it might be angina and now waiting for a CT scan. I now find the symptoms continuously coughing to clear my lungs, out off breath and fatigue. would this be because of the leak in the mitral valve?

You need to go back to your GP. Whilst it may be angina it sounds like there may be an underlying chest infection. They can listen to your chest and, maybe, arrange a chest X-ray. If it is angina they could initially try a GTN spray - if this eases things it points to angina.
It could be something going on with your upper respiratory tract. Then, again, it could be mitral valve regurgitation. Do see your GP to rule out lung issues, but also see your cardiologist. An echocardiogram can shed light on any mitral valve issue. In the meantime, elevating your head and shoulders while sleeping would do no harm and might help.
