Hi, I've simply booked "Assisted passage" and then the printed boarding passes shows me as requiring assistance, all that is at Gatwick. Other airports I don't know about but it works OK at Gatwick every time.

I find that you don't need any other documentation, you ask for assisted boarding telling them if they even ask that you cannot walk far and certainly cannot manage steps to the aircraft. They will then transport you to the aircraft and put you on a platform hoist/lift on the opposite side to normal people boarding. You also often get on first. The last resort is a wheel chair and a personal handler which gets you thro' customs quicker and the boarding process. Unfortunately getting off you are usually last, can't have it always. I find it usually a good service that doesn't cost you any more.

Thats what I do... just fill out the assistance form... no problem in Zurich and Manchester
