Sub cut terbutaline

I don't know whether this is even used anymore. But I've read a little , and I have a few questions for anyone who knows..... I'm currently uncontrolled on uniohillin 400mg bd, monteleukast, relvar, tiotripium respirimat and azithromycin 3 times weekly. Back on continuous pred...only 10 mg, but consultant will increase in 2 weeks if no improvement. I've previously been on pred 40 for 18 months. It doesn't seem to make a huge amount of difference though. Multiple hospital admissions- many to itu/ hdu. I've been turned down for xolair. Very very reluctant to just take more and more pred. The side effects would be ok if the medication helped- but I'm not sure it does. The trade off doesn't seem worth it. I was wondering what next. I don't know what the next 'step up' is. If subcutaneous terbutaline is used still, under what circumstances? Is it reasonable to ask dr if I can go on it? Would I still need other meds? Asthma has changed my life. Is there anything else medication wise that can Be tried? TIA! Sarah

Hi Sarah, S/C terbutaline is usually only prescribed by specialists. I was started on it in 1993, continuous for many years, then tailed off, changed to s/c ventolin instead . now use it as and when needed.

If you aren't responding to pred, maybe you are steroid resistant? Worth asking about this.

You seem to be on hefty doses of Uniphyllin, I used to take that amount but now on 200mg BD.

I am steroid dependent though trying to get down to a low dose as possible. I have been on pred well over 15-20 years now. You seem to be on a similar meds combo to me.

I was turned down for Xolair many years ago as IgE not quite high enough.

There are other newer meds to Xolair now (Monoclonal Antibodies) which may be suitable. some work differently to each other etc.

Can you ask your respiratory consultant to refer you to a specialist tertiary centre?

S/C ventolin worked for me, though now I only use it at home if I am struggling with too many nebs. Though I can only use it for a few days usually due to the cardiac side effects.

(Though may be exacerbated by the menopause at the mo....) Also, it doesn't work for everyone. But worth a try if you can get approval to try it.

No, Tebutaline is Bricanyl, different bronchodilator

Salbutamol is Ventolin... :-)

Thanks for your reply Kate. I am really fed up with it at the moment. You've been through the wringer- what would you suggest as next line of treatment? Is asking about a sub cut trial too drastic at this stage? Or are the ige treatments preferred? I know you're not medical- but maybe you have an 'expert by experience' opinion!

I’ve now been using sub cut terbutaline for just over 4 years. It’s not been the ‘one’ magic answer and I’m still steroid dependant but has certainly helped.
