Feeling at a loss

Ok so been feeling really tight since Wednesday of this week... can barely walk without getting out of breath and even four hourly Nebs don't seem to be easing it as much now. I'm on 40mg of steroids and and this is my fourth day but don't seem to be making an improvement... feeling really unwell and just wondered if anyone had any advise
