should I do a fead back form?

Recently had a bad healthcare experance

not only a case of no wheaze no asthma but also complete lack of deaf awareness by nurses and doctors .

This made things realy bad to the point I would be scared to use the department again.

Should I fill in some sort of feedback form?

Sorry for your bad experience. I think you report it through PALS, but I have no experience of the process so can't really help or offer advice.
