Have just returned from a cruise and on one of the days in a foreign port, it was lovely and hot and upon return to the ship had a beer at 12 noon. Dinner was at 8.30pm and had one glass of wine at 9.00pm making up my 2 units. But surely the 12 noon drink would have dissipated by 9.00pm, so are these limits realistic

Gizmomaven. These alcohol "targets" for folk who take warfarin are not legally binding as in driving but are there as a helpful guide. What is certain is that alcohol will affect your INR and the more alcohol you have the greater the effect. What is also clear is that to get your INR too far away from your target can be very dangerous. Most people who test and manage their own INR do so quite regularly and can do so after a "night out" and adjust their warfarin dose accordingly. They are therefore more free to get on with their normal lives with less dangers and spend more time safely within their target INR range. If you are not doing so I strongly recommend it.
If you don't go out every night, and don't get off your face (drunk) when you do have a drink all should be ok.
I was told by my cardiac surgeon that I could have 6 six pints a day if i wanted too. However he was not recommending that. The point that he made was that it has to be consistent. What you can't do is save up your alcohol and have it all at the weekend. That is the same with all things that you eat and drink whilst on Warfarin. I normally eat a lot of green vegetables and notice a big difference when I go on business trips where eating in hotels I tend to drink more than normal and it is not as easy to get green veg. I also self test regularly to ensure my INR stays in range.
