Would I just work with primary care for testing, or would it be a referral to nutritionist or something, then testing? Where/how have those of you who post about it gotten these tests done?

Depends upon your GP and how flexible your surgery is for testing for deficiency without symptoms. I was lucky as several GPs in my previous surgery were big into nutrition so would do a fair range.
GP surgeries test to look for disease. Nutrition test to prevent disease. As you are in US I would suggest you look for a dual trained Lifestyle (Functional in US) MD. Just google Functional Medicine.
