Has anyone else had bleeding problems when switching from warfarin to xarelto? My INR’s have been all over the map and my Dr. decided to put me on xarelto when my INR came down to 2.2. I stopped the warfarin and started taking 20mg of xarelto a day and after 2 doses I started seeing blood in my urine. Has anyone had similar experiences with this? Thank you.

Warfarin does tend to hang about for a few days after you stop it. What was your inr the day you started xarelto?
Hi Elkhunter, there is a link here to an NHS document that covers this topic cambridgeshireandpeterborou... It appears that there should be a 72 hour gap between stopping Warfarin and starting Rivaroxaban. As JamesKildare stated Warfarin seems to hang about for a few days so you may have too much anticoagulation in your system. If it doesn't clear up in a few days I feel that you should speak to your doctor again. (I am not medically trained in any way so this is just a layman's point of view). Steve
2.2 INR is still within the therapeutic range, so in effect you were still protected by warfarin when you started taking xarelto. When I took warfarin and had to stop for a procedure, I was amazed how long it took for my INR levels to fall. If it doesn't clear quickly, do get in touch with your GP.
My diet is plant based so my INR is all over the place as well. I have an INR self tester and test every 4 days at home. More often of I go skiing. I am still out side the 2.0-3.0 about once a month. A pound of frozen spinach or kale will drop your Warfin INR down really fast. Because I ski and mountain bike my Dr told me I should stay on Wafarin because I could keep it in the low 2s and have less of a bleeding risk than the other anticoagulants.
