Another key recommendation, said Dr Kumbhani, is that when combined with an AC, clopidogrel is the recommended antiplatelet agent for most patients.

"There is limited data for ticagrelor and prasugrel as part of [DAPT], so I think the default P2Y12 inhibitor should really be clopidogrel," said Dr Kumbhani.

For patients with AF undergoing PCI, switching from warfarin to a direct oral AC (DOAC) is recommended. "If you're going to put them on clopidogrel, it's pretty clear that bleeding risk is lowest with clopidogrel plus a DOAC compared with warfarin. There is an app coming out that will make the information we've synthesized and our treatment algorithms easy to access."

It seems that aspirin is finally not to be continued to be recommended bringing US more in line with Europe.

Hi CD, thanks for the link.

It’s hard to unravel this one, but the main thrust seems to be dropping aspirin from the recommended triple therapy antiplatelet regime for patients undergoing percutaneous coronary angioplasty with stenting, who are already on warfarin or DOACs.

Also switching from warfarin to DOAC in the same clinical context, to minimise bleeding risk

That could be controversial.

I guess it will be as so many seem attached to the idea that asprin is therapeutic and carries less risk than DOACs & other anti-platelets.

As my grandmother suffered greatly from affects of asprin I have only ever taken as a prophylactic when travelling long distances.
