Do You Have Dark Urine Caused by Anti-coagulants or Blood Thinners?

I am 72. I was put on Xeralto 20 mg after having 4 cardiac arrests & a pacemaker put in, June 2018. I have occasional AFib episodes, but hardly ever feel them. About 4 months ago I started getting dark urine in the evenings and tests showed blood. Before that, it was always perfectly clear and pale all the time. My urine now is really dark coffee brown at the time I go to bed, but returns to perfectly clear and pale in the morning and all day. I take my Xeralto 20 mg and other meds (bisoprolol 1.25mg, candesartin 12mg, and lipitor) about 3 hours before going to bed. I do have damaged kidneys from when I was younger, but all my kidney function tests have been fine for the last 30 years. GFR was 41 at last screening a couple weeks ago. Have any of you had dark urine (blood in urine) that was traced to your blood thinners or anti-coagulants, and what happened? Or is it a co-incidence hat my urine turns dark brow 3 hours after I take my Xeralto? All comments welcome. thank you!

Not personally but I am sure I have seen this reported before. Maybe others can confirm. Have you tried searching blood in urine?

Generally the body well hydrated will only show up darker urine am subject to your Kidneys / Liver having been starved of fluids overnight. Hence the term fast.

Therefore meds etc will be colorful.

Causes of blood in urine. Blood in your urine could come from anywhere in the urinary tract - the bladder, kidneys or urethra (the tube that carries pee out of the body). If you have other symptoms, this might give you an idea of the cause. Don't self-diagnose - see a GP if you think it's blood in your urine. Blood in urine must be checked out because it can be a sign of cancer. This is easier to treat if it's found early.

I've already done a lot of research and had tests and discussions with my doctor. What is odd about this case is that the blood is only there in the evening, appearing shortly after I take my meds. And only started after I'd already been on these same meds for 19 months. I drink plenty of water and except for this short period of time in the late evening my urine is pale and clear enough to read through. I am just asking if anybody else has experienced anything like this.

From what my wife was recently told, blood in the urine can be normal and especially when we are older. It can be a sign of something unwanted, but once these causes have been ruled out by scans and tests, should be ignored unless it again changes and or excessive.

It seems that you fit into the "safe" category, so can breathe easy!

Yes, Xarelto!

I take xarelto and have never had this. The odd thing is that the urine clears up overnight and all day. Becsuse you had blood in your urine before, you probably need to have it retested. Another reason urine goes dark can be liver issues but it wouldnt go back and forth.

When you say your kidney tests are OK, is that for creatine? Coffee coloured urine when taking amiodarone or dronedarone is a sign that it's not tolerated. Assuming you are male, do you have an enlarged prostate?

To answer your question, I'm 73, I have had coffee coloured urine and, my enlarged prostate (thought to be non-aggessive prostate cancer) bleeds from time to time usually caused by hard 10km rowing machine sessions for at least three days in a row.

Hello goldey

I have had the same symptom - blood in the urine. It began around 3 weeks after I started taking 20 mg of Rivaroxaban daily. Happens just occasionally though, mostly after exercising. My cause was found to be a kidney stone that was moving around scraping the inside of the kidney. The stone is otherwise painless and I do not have any other issues with it except the occasional blood in urine. However...never had blood in the urine before starting on Rivaroxaban (Xarelto).

Hi Goldey. I'm currently on Xarelto after a femoral vein DVT and two PEs. That was a year ago and I'm finding the side effects of the different anticoagulants debilitating so I'm weaning myself off them. Xarelto is the one I'm on now, after using enoxaparin and apixaban. All of these meds made my urine brownish as if tinged with blood, apixaban most notably - but never as dark as you describe it. They all also caused joint inflammation and bone loss, and apixaban caused anhedonia. Xarelto has caused mild liver issues and hair loss.

Plasma concentration of Xarelto peaks around three hours after you take it, so it looks like more than a coincidence that this is when you notice this issue. You're probably correct about what's happening. Google "rivaroxaban blood in urine". One researcher has "recommended dose adjustments or temporary breaks from antithrombotics if they are causing hematuria and potentially doing a more thorough investigation to determine the underlying cause of the problem." I'd speak to my doc again if I were you. Good luck.
