AF again!

1st time AF has reared it’s ugly head after ablation 5 years ago. I think it was brought on by being in a very stressful situation.I have just come home after 3 nights on the cardiology unit trying to chemically cardiovert me. Tried digoxin and upped my bisoprolol to twice a day to no effect. Remained very breathless on exertion. I suggested IV fleicanide as it has been very beneficial in the past. Cardiologist agreed but gave me 150mgs orally instead. I didn’t think it had worked after 5 hours but when I awoke this morning I was in sinus rhythm. I have come home with fleicanide as a pill in the pocket if I go into AF again. Also been referred back to my EP to discuss if I need a further ablation or hopefully I won’t have any more episodes. Anybody on this forum take fleicanide as a PIP if so has it been successful each time of taking

Yes, Ive taken it for 5 yr now and it has worked every time. My longest episode was my last one about 19 mo ago. It was also my hardest lasting 12 hours. EP convinced me to take it twice a day for a few months and I did, then came off it without an issue. If I had frequent a fib I would take it regularly but for now, this seems to b a good plan for me.

Well, I guess it depends how you quantify success......since my second ablation, I started to get episodes after just under a year. Flecainide as a PiP has stopped them within an hour and they have not been as unpleasant but I am on the list for a third but doubt it will happen until next summer. Stress seems to be an issue and I’m having my thyroid looked at too......good luck.....

Yes I took for a while & in worked in the early days when episodes were infrequent.

I took 300mgs each time.

I didn't realise Flec was available in 300mg dose - I couldn't cope with 100x2 a day! Though they have told me i can take a 50mg dose x3 a day if needed. Thanks for sharing the information

300mgs is 3x100mg tablets and is the maximum dose in any 24 hours but taken as a PIP it stopped my AF in the early days within an hour.

I take my Flec as PIP as needed. Leaves with me with a terrible body tiredness hangover for 2-3 days so I am lucky that I have not had to use it that much. Normally brings me back to NSR within a few hours.

That must have been very upsetting for you especially after being 5years free of af - that is a long time. Do you know what your heart rate was you must have been quite poorly seeing you were 3nights in hospital.

I had 1st ablation 2months ago & was put on 100mg Flec to take x2 a day until my follow up with cardiologist - but ! the dose was too high as it caused me to feel very sick & faint so it was halved. 300mg is quite a high dose but shows how different we all are in how we react is that dose to be taken in one or, spread across the day?

Following cardioversion this week i have come off flec for now as they think?? Its been causing ectopics. & now told to use it as a pip. Thank you for sharing your experience it helps understand just how difficult af is to deal with. Hope you feeling better.

I am surprised you had to keep taking the flecanaide - I was always told it was a drug to get me back into sinus rhythm but not for daily use. Strange that different EPs have such different views!

It can be both - lower doses for daily use, and usually a high 300mg dose used for PIP to stop an episode.

I took flecanaide as a PIP for some 15 years - sometimes had to have it IV as I was being sick - which tended to be the trigger for ty r AF . It always worked until

It didn’t then I had to have an ablation

Good luck

My normal medication is 2x100mg flecanaide daily but if I have a fast PAF episode I have to take an additional 100mg for up to 4 days, it always seems to work within a couple of days. It seems to work for me.

The reason I have to take 200 mg Flec daily is that I can feel the SVT or AF episodes starting every day and the Flec kicking in to halt it after a few seconds.

Flecainide is my saviour and I stop short of praying to the pills at night.

Took it three times in as many weeks when I went into Afib in August...last two months I have been on Flecainide twice a day 100 mg to review in February. Heart has been absolutely fine even when it underwent a stress echocardiogram. Part of me resents being on meds possibly for life but better than the alternative and apart from that don’t have to take anything else. Alcohol is definitely an irritant so I am committed to low alcohol beers which seem fine. Like everyone else (most of whom have far worse stories) I do struggle to understand how my life has changed albeit subtly since August 2018 when this “journey” started!

I use flecainide very successfully as a PIP, sometimes with added (low dose) bisoprolol and episodes rarely last more than two hours. It didn't work at preventing episodes as it is supposed to which I tried for a while but we are all different and hopefully use as a PIP will work for you as it does for me. Good luck. It also gives you the confidence that yu have some control over the thing!!

I don't take any pills, but this reduces the severity of my afib and often stops it. Give it a try and see if it works for you:

------------------------------------

After 9 years of trying different foods and logging EVERYTHING I ate, I found sugar (and to a lesser degree, salt – i.e. dehydration) was triggering my Afib. Doctors don't want to hear this - there is no money in telling patients to eat less sugar. Each person has a different sugar threshold - and it changes as you get older, so you need to count every gram of sugar you eat every day (including natural sugars in fruits, etc.). My tolerance level was 190 grams of sugar per day 8 years ago, 85 grams a year and a half ago, and 60 grams today, so AFIB episodes are more frequent and last longer (this is why all doctors agree that afib gets worse as you get older). If you keep your intake of sugar below your threshold level your AFIB will not happen again (easier said than done of course). It's not the food - it's the sugar (or salt - see below) IN the food that's causing your problems. Try it and you will see - should only take you 1 or 2 months of trial-and-error to find your threshold level. And for the record - ALL sugars are treated the same (honey, refined, agave, natural sugars in fruits, etc.). I successfully triggered AFIB by eating a bunch of plums and peaches one day just to test it out. In addition, I have noticed that moderate (afternoon) exercise (7-mile bike ride or 5-mile hike in the park) often puts my Afib heart back in to normal rhythm a couple hours later. Don’t know why – perhaps you burn off the excess sugars in your blood/muscles or sweat out excess salt?? I also found that strenuous exercise does no good – perhaps you make yourself dehydrated??

I'm pretty sure that Afib is caused by a gland(s) - like the Pancreas, Thyroid (sends signals to the heart to increase speed or strength of beat), Adrenal Gland (sends signals to increase heart rate), Sympathetic Nerve (increases heart rate) or Vagus Nerve (decreases heart rate), Hypothalamus Gland or others - or an organ that, in our old age, is not working well anymore and excess sugar or dehydration is causing them to send mixed signals to the heart - for example telling the heart to beat fast and slow at the same time - which causes it to skip beats, etc. I can't prove that (and neither can my doctors), but I have a very strong suspicion that that is the root cause of our Afib problems. I am working on this with a Nutritionist and hope to get some definitive proof in a few months.

Also, in addition to sugar, if you are dehydrated - this will trigger AFIB as well. It seems (but I have no proof of this) that a little uptick of salt in your blood is being treated the same as an uptick of sugar - both cause AFIB episodes. (I’m not a doctor – it may be the sugar in your muscles/organs and not in your blood, don’t know). In any case you have to keep hydrated, and not eat too much salt. The root problem is that our bodies are not processing sugar/salt properly and no doctor knows why, but the AFIB seems to be a symptom of this and not the primary problem, but medicine is not advanced enough to know the core reason that causes AFIB at this time. You can have a healthy heart and still have Afib – something inside us is triggering it when we eat too much sugar or get (even a little) dehydrated. Find out the core reason for this and you will be a millionaire and make the cover of Time Magazine! Good luck! - Rick Hyer
