I haven’t seen any evidence, but my suspicion is that DOACs are safer then Warfarin, but less effective. The studies I have seen have compared DOACs with Warfarin where INR is in range less than 60% of the time, whereas it is relatively easy to achieve over 90% with self monitoring and self dosing.

Well controlled is the critical phrase, Most people who rely on INRStar programs will fall out of range far too easily as there is no trend recognition protocol involved.

Does warfarin offer a higher degree of stroke protection? I would think so because you have a monitoring system in place unlike the newer drugs.

For me one DOAC wasn't as safe as Warfarin, and rather than be "bitten" a second time by taking a different DOAC I reverted to Warfarin. The lack of safety was not in relation to anti-coagulation but to a very rare side effect.
