took a double dose of 5 tablets warfarin included.missed all next day.hope im doing right thing.

I have taken a double Apixaban dose in error and tel. edt he nhs helpline.No problem.Apixaban is occasionally given as a double dose anyway they said.Not sur if this is helpful.

In the circumstances you could have done little else.

I would contact your doctor tomorrow and get an INR test done to put your mind at rest.

Don't play with sharp objects today.

it won't take effect yet- agree with others to get INR checked and tell clinic as they should know when effect will come in.

You really should have an INR check and advice from your GP - buy yourself a tablet box showing days of the week and put your tablets in there religiously then you will soon see whether or not you have taken that days medication.

I thought the fun of choosing the colours to go in the tablet box was the best part of taking warfarin. It's better if you are on a high dose, like 10, which can be two pinks, or pink, plus blue and two browns, or three blues and a brown. A little mental challenge to keep the packets level pegging!

Once again, I feel intense disappointment. It is very common to forget a dose, and there is standard advice. But there is no advice about when you get confused, and take it twice. There should be an FAQ somewhere for questions like this.

OK, as a self doser for over 23 years. These are the options.

1. Miss the next day, then carry on as normal. There is a large amount of delayed action, and within two weeks things should have settled to normal. There is no need whatsoever to report, ask questions, and do anything. Intervention could easily make things worse.

2. What I did last year, was to 'step'. the next few days being 3, 3, 4, then 5, or better: 3, 4, then 5. This stepping reduces the swings. The subtle point is that you do NOT want to totally miss the 5mg to compensate for a double dose, instead, because of excretion of 10mg being higher than 2*5mg then you aim to cut around 70% in the next few days.

3. Now, another nuance. if you have a recent INR, you will know if you are at the low end, or at the high end. If you are at the low end of your range, then I would take 4, 4, then 5. The double dose will simply have given you a small boost in the reservoir level. But if your INR is probably high, then you probably want to step more dramatically, eg 3, 3, 4, 5.

I doubt very much if your INR clinic staff have thought much about this, though I did once know a haematolgist who would think at this level of nuance. If you know one, perhaps you could insist on talking to them, and not the others who you know are a bit befuddled and simplistic.

Life gets even more complicated. Several years ago, I was not sure: had I doubled the dose, or not? I had no easy access to testing. So I had to work on the balance of probabilities. I reasoned, it is rarely dangerous to miss a day then carry on as normal. So that is what I did.

Apixaban effect lasts 12 hours, therefore your mistake will last 12 hours. Just do not make the same mistake again.
