Back in AF sometime overnight. confirmed by the Alivecor this morning :(

How disappointing for you. As you did achieve NSR for a very short while is there any chance that an ablation might work ? I know previously you have said that it wouldn't because you were permanent but does this change things?

It's not that ablation won't work, it's that being persistent for so long it has a much lower success rate, as I understand it the electrical impluses have now found their own (and wrong) pathways, and the longer they use those wrong pathways the harder they are to shift back to the correct ones.

Add to that probable fibrosis, and some degree of enlargement (cardiomyopathy) and I understand that catheter ablation for me might be as low as 20% success rate. Surgical ablation would be higher, but is a much more serious op with it's own risks (and rewards as they can carry out a LAAO at the same time)

So then do I risk ablation, with it's risks of course, not least of which for me might be a much worsening of the symptoms, or do I carry on regardless with what really are only very mild symptoms, EXCEPT the ruddy arrythmia?

Oh Ian, what a quandary .Having the LAAO done would be a huge plus but the risks of surgical ablation versus success are a tough one and the success rate for you of catheter ablation are low ,I don't know what I would do. Pros and Cons to everything.

I am so sorry the cardioversion didn't last but at least you did feel what NSR was like even if it was only for a few short hours.

Well as has been said you can be pt into NSR so an ablation is a possibility so good progress.

Oh RATS!

What a quandary for you, made even more difficult by the fact that you feel o.k. in sinus rhythm. At least if your quality of life was poor in A.F. the decision would be easier. Do you trust your E.P.? If so, maybe now is the time to be led by him although even he may be reluctant to give an opinion as so many seem to sit on the fence. I asked mine to tell me what he would do as a human being not a doctor and he told me he would have an ablation so that was good enough for me.

As ablation only has a 20% success rate for you I think I would seriously consider a surgical ablation as I have heard the success rate is high but it is a very very personal decision.

I hope we have been of some help to you as you are always so helpful and also practical when we have decisions to make. I will be thinking of you.

Sorry to hear that Ian. What is your gut telling you about the way forward. Once you get past the disappointment I would think long and hard about the way forward and be guided by your gut feeling in consultation with your EP's facts and figures.

Sorry to hear all that, sounds like a difficult decision for you. Having watched the TV programme about costs and rationing of procedures on the NHS I am sometimes surprised to see how 'easy' (please nobody shout at me!) it seems to be to get listed for an ablation so I guess it must be considered cost effective?

So disappointing of course Ian, but it's good to know that this time, at least you stayed in rhythm longer than before. Difficult decisions to be made I know, but you will get good advice to help you move forward.....see you in April, John

Just how risky is the surgical option Ian? Statistically, more so than catheter of course but you're not a fragile old man (unless we've all been mightily deceived!) and this is 2015 with all its advances in technology and medical expertise. The rewards, as you recognise, could be enormous.

Hi I an know how you feel went in to AF on Monday ended up in A&E for 7hrs had lasted 6 wks in sinus rythem only recently diagnosed with this .Take care and wishing you back into sinus- if only that simple!

Trying to be positive Ian. I have had 2 ablations and was told that I would have to wait 3 months for scarring to occur (crust for want of a better word) before they could say for sure. I was warned that AF could be worse or better during this period. I am keeping my fingers crossed for you.

Oh how disappointing Ian, AF not only messes with your heart it also messes with your head by only giving you a taster of NSR. Am not sure what is worse, being in AF or waiting for it to happen. I hope you can get an effective treatment plan with your EP (who is this by the way). I know you have some big decisions to make, take your time and I am sure you will make the right one. Know how you feel . I have persistent AF and had 6 months in NSR , went back into A fib 2 weeks ago , it just happens I've been told , no reason , just woke up to my heart jumping around in my chest , hey ho . Back on Digoxin and waiting to see my cardio next month . Meanwhile I'm just getting on with life refusing to let it rule me
