John I always get confused is it 20% of the list price, the insurance price or what Medicare pays? My wife tells me she always tips her hairdressers in cash rather than plastic. That way they do not pay taxes.
TIPS TO FOLLOW:
Generally 15% is the normal tip, but if the service was excellent you can give 20% if you wish.
Cash is best as a tip since "the service provider" can hide that amount (under the table so to speak) from reporting on their taxes. (But not too much cause the IRS knows this happens, note; IRS = I Run Scared).
If you dislike the service you received then don't leave a tip.
True story, when I was affiliated with Visa.... they had a customer who was really angry at the service he received at a restaurant so he left a MINUS tip which he subtracted from the total bill. Visa didn't want to mess with their Merchant so they "ate the bill" as is.
Tipping medical employees is unheard of. Maybe donuts or cookies for the staff and maybe a bottle of wine for your Doctor(s). No cash.
Tip amount is generally based upon gross amount(s).
Reporting tips on your w2 or 1099 forms may be a thing of the past, based upon new tax law(s).
