I’m on Enzalutamide (Xtandi) monotherapy at the moment which seems to work fine and the side effects are manageable (I work out pretty hard almost every day to keep them abay). I still have some libido and I still “function” somewhat in bed with some help, but would do anything to increase my libido as it’s very important to my QoL.
I’ve read Darolutamide (Nubeqa) doesn’t pass the blood-brain barrier and therefore doesn’t affect libido as much, so I plan to talk to my doctor and ask if switching to Darolutamide monotherapy instead would be a possibility for me.
Anyone here on Darolutamide monotherapy?

I am 74 and on Nubeqa ( Darolutamide) with Leuprolide and I was considering stopping both in 3 months after 1 year on it if my PSA continues undetectable.
Interesting to hear that you have some libido when the Nubeqa drops the testosterone to almost zero,
I still had libido (although very lowered) when I was on Lupron + Zytiga for two years. Now on Xtandi, which doesn’t affect me as much (yet at least), but I’m still interested to see if there are even “better” alternatives, hence my question about Darolutamide.
Nubeqa does not drop testosterone.Nubeqa works to block the effects of testosterone from reaching cancer cells. It belongs to a class of medicines called androgen receptor blockers, also known as anti-androgens.
After three years of monotherapy, I changed bicalutamide to darolutamide. After next half a year, I added a small dose of Orgovyx, which increased the effectiveness of monotherapy and solved the problem of gynecomastia.
I had not gynecomastia because of radiation on brest 3x5 Gy. Howewer I had brest tenderness. Nubeqa double your testosteron. Low dose of Orgovyx can reduce , normalize level of testosteron.
I tried to switch from Erleada to Nebequa (Apalutamide to Darolutamide and couldn't get insurance to cover it because it is not approved for use in Hormone Sensitive PC unless you have taken or are taking docataxel. The cost without coverage was over $16,000 a month.
No no no. The letter U does not follow the letter Q in Nubeqa but it does follow the Letter N instead of the letter E. And the words "Big Bucks" follow the word Nubeqa.
