LIVER FAILING WHILE ON ABIRTERONE

Boys, I finally got my home city to treat me, notwithstanding I had to go to India and Vancouver canter centres, both of whom approved me for abirterone/pred combo. So, I am now three months into the treatment of air 2x500 mg. pred 2x5 mg.

What has happened in the past month,, is my liver is showing signs of failing, so I will told to stop both drugs for a few weeks and see if my liver comes back. If it does, then he suggested we go back at 1/2 dosage.

Please give me your thoughts. thanks and LIVE LARGE MEN, you only have so many days left so do not waste them on trivial things and issues, make up a bucket list and do as many as you can.

After 26 years, I am not quite ready to quit!!!

I had the same initial reaction to Zytiga and Prednisone with my liver. I did the reduction of 50% for the Zytiga for about 6 weeks and my liver improves enough to go back to full dosage.

thanks, my Oncol took me right off for two weeks then if livervrecivers will go back at 1/2 dosage - which might be enough !

Billyboy 3. That situation happened to me 1000gm Zytiga 1 hour before food then 5gm pred daily when I started treatment Liver enzymes after 3 months. Left off dosage for one month then went back 500gms daily been fine since and after onto PSA undetectable

I have been using 500 mg of Zytiga and 5 mg of prednisone for the last 5 years or more. I did originally start with the full dose, but cut it as I was worried about the long term side effects.

I take the zytiga with food, egg on toast and a yoghurt, the prednisone I take about and hour before. It has worked well for all these years and I hope it continues to. No adverse effects.

26 years, grab the rest buddy !

Zytiga caused my ALT and AST to rise to 10x the upper normal limit in 6 weeks. Oncologist stopped Zytiga for about 6 weeks until liver numbers returned to normal. Rechallenged Zytiga at half dose, numbers immediately rose again. Stopped Zytiga permanently to avoid permanent liver damage.

Orgovyx only for 12 months ending mid August along with focal proton treatment on sites that are detected on a psma approximately every 3 years.

Liver toxicity is listed by the zytiga manufacturer as a common side effect and for that reason during initial treatment they recommend very frequent blood testing. I was one of those that could not tolerate the drug. I do not drink any alcohol either so that was not a issue.

After 8 months of Zytiga and my ALT and AST liver numbers remain in the middle of the normal range. Liver is also processing alcohol. But I am concerned about the long term effects of abiraterone (Zytiga) and will ask my MO about reducing to half dose, or changing to darolutemide (Nubeqa) now that the ARANOTE trial has proved it OK in doublet with an ADT like my Orgovyx (relugolix).

Dosages are: Orgovyx 120 mg, Abiraterone 1000 mg, Prednisone 10 mg.

what does liver failure mean. Elevated SGOT AND SGPT or signs of liver failure like low albumin and low clotting factors. If it is just elevated liver tests that just means it is inflamed not failing. Will go away when you stop the drug.

I have been on 500 mg for a year and my T is extremely low and PSA is undetectable.

I had the reaction with Bicalutamide and my liver values were not good. So they took me off it and i get pamorelin shots every 6 months. Have two more left. PSA holding below <0.1. As my profile note HDR Brachy about a year ago and before 25 EBRT....they consider me cured but are you ever cured actually after going through the experience? in any case so far so good not knowing what tomorrow brings so live every moment in the now and excercise..

I talked and talked until my MO put me on 250 with a low fat 500 calorie breakfast. He did, my PSA remains 0.1 for 6 months now. I tolerate it better, plus only 5mg of prednisone. The reason that the studies were not finished on this, instead of 1,000 mg daily, are obvious. The drug maker makes less profit. WHY would they NOT want to know they could cut back? JMO, but they did initial studies on this. Go forth and GET BETTER. I have been doing good for 2 years now. I did chemo...hated it, came out and my bone pain is GONE... stage 4 prostate, mets to ribs.

I was prescribed 1000 mg Abi on an empty stomach and I took 250 with low-fat breakfast from day one. First Psa test my Psa dropped from 20 to 5 in three weeks. At this point I told him and he was good with it. MO retired and first thing new MO asks “is it about the cost”. So I’m thinking to myself , why would you take 4 of these pills if one will do the same.

Maybe you could change to Nubeqa and ADT. It might not have the same effect on your liver.

That is plan b but might become plan a. Will see in a week if my liver bounces back! That predisone is powerful drug!!

I suggest you research curcumin for cleansing the liver. I take Abi with 5mg prednisone, 400 mg of Celebrex, atorvastatin and a few other drugs. Many of these drugs are to be hard on the liver/kidneys. Before I started curcumin my kidney flow was high 80s which is fine but bilirubin was always too high. Now my ast, alt and alp are all in normal range and kidney flow rate is 117+.

Hey your that guy. The guy who appears with some common sense outlook on our challenges whenever you reply in the forum. 26 years! Keep it up Brother Billy.

I think keeping track of your labs when you go back on half dose abi you'll do fine. If not at least the frequent labs will let you know before there is much damage/trouble but you'll be good I think. I dont have to tell a brother like you this but just sayin dont think about the rare possible "uglies". Your frequent lab tests will keep you away from anything severe happening imho.

Way to go man.
