Is Denusomab given just for bone strength or also to prevent spread of Bone metastases

Dad ,83 , has many bone mets and also has osteoporosis in some places.

One MO prescribed Denusomab 60 mg once in 6 months which he has taken one shot of.

Another MO I consulted prescribed 120 mg Denusomab to be taken every month.

What dose do most people with bone mets and weak bones take ?

Does Denusomab also help in preventing spread of bone mets ?

This is a really good question Tinkudi. I am not on the drug yet. I have another bone density scan coming up this fall.

No, it does not prevent spread of bone metastases. That was proved in 2 large randomized trials.

Thanks Allen. 😊.

Which dose is the correct one- 60 mg once in 6 months or 120 mg monthly ?

Hi Tall Allen,

My MO has asked my opinion on resuming Xgeva after stopping for an ONJ scare -- it wasn't ONJ, just some TMJ issue.

I have not been able to find the post where you linked to the two trails you mentioned above. f.y.i. I am post ADT, chemo and Pluvicto with DEXA scores well above +1 for both hips (mild osteopenia in spine) -- MO wants Xgeva every 3 months as he believes it is SOC.

Would you please repost the links so that I can share with my MO? As you know, quoting some guy, no matter how respected, from the internet means nothing to a doctor.

Thanks in advance.

Xgeva is every 3 weeks.

The trials that proved Zometa does not slow metastases or increase survival are these:

I take 120 mg every three months, since Jan 2020. I took a year of in 2022-23 to do some dental work. The "correct" dosage seems to be the individual doctor's opinion.

I was taking the 120 every 3 months. Many MO's have learned that monthly injections had little benefit over the 3 month, and a higher incidence of side effects.

Tall Allen has the facts.

My 2 ¢: In my case my retiring MO said I didn't need Xgeva (high strength monthly or quarterly shots) or Prolia (low strength 6 month shots) but my new one said I did need Xgeva.

So I did a consult with an MO at Memorial Sloan Kettering in New York City and she said she felt it had a therapeutic effect beyond strengthening bones. So I am back on it -- well on hold until a dental issue is resolved.

My most recent DEXA showed very strong hips and only mild osteopenia in my lower back, so there is no need for Xgeva for my bone strength.
