PSA started rising quickly after dose 5.

it was not caused by the Pluvicto - the drug stopped the cancer for a while, but then it stopped working and the cancer began its relentless march again. So we knew it stopped working when PSA began to rise quickly - at same rate as before the drug.

had two rounds of Pluvicto. Hemoglobin and platelets dropped too low to continue.

I had 5 rounds, the sixth was held off. Had very dry mouth and eye SE, blood levels were still ok for 6th. PSA was 68.37 at start of treatment went down to 1.3. MO thought risk of 6th outweighed benefit. About 9 months after last infusion PSA started to slowly rise. Scans showed two hot spots on bone, SBRT planned for next month to radiate those two spots.

Stopped after 3. Rapid PSA rise after it had dropped. Scans showed progression.

Husband completed all 6 treatments. He tolerated it well but it didn't have lasting effects. Began cabitaxel about 5 months after last Pluvicto treatment.

He's had testing done 5 times with no actionable results.

hubby got three treatments but his body gave up before any more. His PSA went down to two digits for a while then went back up to triple digits.

Same thing with my husband. He was in so much pain and so sick. His PSA had dropped to 16 but went back up higher, 112.

I went into massive bone marrow and renal failure after fifth dose.It has been 1.5 years. PSA remains undetectable, but am on dialysis for renal damage. Bone marrow resolved after multiple transfusions.

I was treated last year in Austria, Dr's plan was for 3 initial treatments. Still hormone sensitive. PSA was 168, had metastases to bones in pelvis spine and ribs. Concurrent first use of Hormone blocker, OrgoVyx. First dose was alpha particle, PSA declined to 7 after 3 weeks. 2nd and 3rd doses Lu-177(Pluvicto), PSA 2.0. Doctor wanted to wait. Had PSMA scan 3 months after 3rd dose, PSA .09. Orgovyx discontinued. Awaiting next PSMA scan as PSA has risen to 5. If cancer is still PSMA avid, will repeat treatment. Apparently hormone sensitivity can be a plus for effect of LU-177 and discontinuing Orgovyx may help preserve this.

On the day of the 6th treatment, the presiding nuclear medicine doctor discussed with me my borderline neutrophil counts. I could have pushed forward with the 6th if I wanted (my choice), but because my PSA was already undetectable for months and bone mets either gone or low grade bone remodeling, we delayed the treatment to watch the neutrophils. They dipped even further over the next few weeks so we ended up cancelling. It was part of the PMAddition Trial and I think too much time passed since the 5th treatment anyway. Months later PSA is still undetectable, neutrophils have bounced back and bone scans look good with only a small area of residual very low grade activity of bone remodeling so it seems like the right decision.

For me, treatment #5 took me to undetectable so we stopped. I’m now doing a rechallenge 4 years later with excellent success.
