try to do radiation with provenge for better results.
Nine years ago I went abroad to consider Cyberknife and HIFU - former was just too new in US and latter not available. The fine docs I consulted, based on careful study of my mpMRI findings, pathology and genomic testing, felt neither were in my interest.
Curious if these investigative methods and additional ones available today are part of your doc's recommendations.?
Where is your visible cancer now? Which scans did you have?
Ilium, Sacrum, L4. CT, Bone and MRI's.
Maybe you should get a psma pet ct scan in order to confirm all these Mets and hopefully you don't have more otherwise you should stick to a global treatment if you have even more confirmed visible Mets. I would not do radiation before finding an effective global treatment. I would maybe do spot radiation if after a new global treatment you still have visible Mets remaining on the psma or bone scans. You can't radiate all these Mets. You should first try to shrink them and only radiate the ones remaining after a new global treatment.I hope that you may end up getting rid of all visible Mets without a need for radiation and use radiation only if is it absolutely necessary. Unfortunately it is not realistic to radiate too many Mets. You should save the radiation for later if you can.
Had CT and MRI a year ago. Pelvic region has plenty of cancer cells in both ilium and sacrum. Also L5 met. Plan is to see what X-Ray shows and then likely Bone Scan followed by PET scan as needed.

With so many on the CT visible Mets you need a global treatment. I don't see how radiation alone could help you long term. Definitely do provenge as soon as possible.
Did you have genetic testing?
Maybe you could try clinical trials?
Which hospital is taking care of your cancer?
Could you ask for second opinion?

Preparing to do genetic testing. Have had the saliva testing, but going to look at biopsy testing of cancer cells next.
Second opinion is not a problem. We have others that we do consult with.
UCSF is one of the two affiliated hospitals we work with. Clinical trials are available as needed.
The good thing is that it looks like that Enzalutamide is still working for you at this moment. You should really have proper scans to see what to do. Like bone scan etc. you should not rely on the PSA alone.
You will need baseline scans in order to compare them in the future in order to check the progression of you cancer and the effectiveness of your treatments. As I said PSA alone is not enough for you. Therefore psma pet/ ct scan with contrast, FDG pet scan etc. Hopefully all of them will reflect that your PSA is a good indication. Sometimes the bone scan will show progression despite low PSA.
