After 2 years on ADT I must go to a stronger medicine. I am castrate resistant with one small met by the pelvis. Doc suggests Xthandi but I would appreciate any suggestions on anything easier. The SE scare the hell out of me

Xthandi or nubeqa
Hello,
I am not sure why the side effects of Xtandi (enzalutamide) "scare the hell out of you" when you haven't even tried the drug. Anecdotally, I took Xtandi for nearly six years before it started to fail. The side effects, for me, were minimal. Some fatigue, a little peripheral neuropathy which on reflection might have been a side effect of back surgery. Nothing serious and nothing I couldn't handle.
My experience is that Xtandi has the worst side effects of the testosterone blockers. Bicalutamide the least. Xtandi crosses the blood-brain barrier and it did a very unpleasnt job on my brain. I just laid around all day. Did not feel like doing anything, Slept a lot.
I just went off abiraterone + prednisone after a six month regimen. It kept my PSA undetectable. The side effects were much more bearable than any other program I've been on.
Likely the more aggressive programs such as leuprolide + abiraterone are more effective but unfortunately greater treatment efficacy seems to go hand in hand with greater side effects. At least for me. When I have to go back on treatment I hope to take only abiraterone /prednisone again. But if the PSA goes back up too soon and/or the PSMA scan shows any new lesions I'll probably ask for leuprolide + abiraterone. I will be discussing this with Dr. Sartor in June when I see him next.

Hey Man. I appreciate that you have the balls to express your feelings. Almost everything about our attempts to slay this beast is scary. Don't be intimidated by the hard assed comments that some people will offer you.
I did very well on apalutamide plus Lupron for two years.
like one report below I had horrendous side effects from enzalutamide. The worst were on the brain. Headaches which ended with me in the unit on morphine PLUS severe hypertension (probable PRES) PLUS high blood triglycerides weight gain and cognitive blunting.
Make sure you are fully counselled on all the side effects and their frequency and what to do if you get any.
Also ask what the effect of the drug is on “all cause deaths” in a 79 yr old. My reading is bugger all.
