I am confused about the differences between the two scanning procedures and the sensitivity of finding new metastases.

An MRI sees tissue abnormalities but does not detect cancer. A PSMA measures PSMA which is expressed by cancer tumors.

An MRI doesn't detect cancer? My oncologist does an MRI of the hip/prostate annually and the reports seem to comment on lesions, etc. The language is indirect but all my scan reports seem to use indirect language, even PET reports and CT reports.

PET scans are usually combined with simultaneous CT scans for image resolution. Siemens has a device that simultaneously provides a PET scan and an MRI. This enables greater image resolution and the detection of smaller metastases than is possible with a PET/CT. (GE and Philips manufacture dual scanners rather than an integrated single scanner). The PET/MRI exposes the patient to a much lower dose of ionizing radiation than the PET/CT. These devices are expensive, and are only available at a few large tertiary care facilities. In one PET/CT vs. PET/MRI comparison (see link below) using Ga-68-PSMA, the PET/MRI was able to detect 42% more metastases in recurrent patients, 10% more lymph node metastases, and 21% more bone metastases. In the US, PET/MRIs are in use at Mass General, Johns Hopkins, Stanford, UCSF, Washington University, Cleveland Clinic, and Memorial Sloan Kettering and several others.

I was enrolled in an SBRT-MRI Clinical Trial at Dana Farber for Mets in my lymph nodes and abdominal wall.

SBRT-MRI Trial using ViewRay machine was just cancelled, and they unenrolled the Phase I patients because ViewRay, which has gone bankrupt, cannot be used after Oct 25th legally, I guess.

My RO bagged out, and I have about had it with Dana Farber, after many other issues. Might be looking at trying MD Anderson, or just have good Scotch and Bourbons for a while. Hell, I’m about to be 77, how much longer do I have left anyway? Spend that limited time in hormonal and chemo misery, or go live that time my way?

Anyway - SBRT with MRI using a ViewRay Linear Accelerator might not be an option at this point. FYI

You can still get SBRT from Irving Kaplan at Beth Israel Deaconess. But the OP was asking about using PET/MRI as a diagnostic tool, not MRI-guided treatment.

Thanks TA. I am looking through other trials here, and considering really looking at Proton therapy. Might also change hospitals, checking out MD Anderson in Houston possibly.

Dana Farber Cancer Institute and Brigham and Women’s Hospitals are in the middle of a seriously messy operational divorce. DFCI “blindsided” Brighams with a switch to a partnership with Beth Israel Deaconess to build a standalone inpatient hospital/labs next door. Obviously it’s all about money. Theoretically patient care will not change. Suuuure!
