I received my 1st dose of Pluvicto 2 weeks ago and have had a severe pain flare since then. I am wondering if anyone else had a PSA in the 1500’s with extensive bone Mets before starting as well as the BRACA 2 gene??

Have BRCA2. After 4 treatments of Pluvicto my bone marrow became weakened beyond the point of no return. Dangerously low blood counts resulting in no viable treatment options.
Did you have both FDG and PSMA PET CT prior to starting with Pluvicto?
My PSA was only 2 (reduced 0.2) but have extensive small met's. I have finished 4 as part of Eclipse trial and developed burning, stabbing pain in my upper spine region and between my scapula's. We think it may be related to my T-6 that had my largest lesion. Taking Gabapentin, Tramadol and Ibuprofen,
I do not have BRACA2. My PSA was only 99.7 but I do have extensive bone mets. My pelvis and upper femurs are essentially just tumors along with a bunch of vertebrae and ribs.
When I consulted a Medical Oncologist at Memorial Slone Kettering about treatment options she agreed with my local MO that Pluvicto was my only real option. She also said I was a high risk for "rebound pain." She recommended I get steroids for the first week. I have had no pain, just a little upset gut.
My Radiation Oncologist was very concerned after her experience treating me with Xrays at the beginning of the year, so I think she may have over done the steroids as I was ready to remodel the entire neighborhood that week and it's taken another week to get back to normal (whatever that is).
You should bring this up with your team to see if this approach might be right for you.
My PSA was 1707 but I went a total different route, biopsy, adt shots, bone infusion, some more different meds until the Chemo creeper (didn’t wrk, but w/o it I couldn’t sign in for pluvicto), started on Abi, brought my #’s down to 11, but later started climbing, just in time for my first LU177 infusion, just had my 3rd , not really bad SE on all 3 infusions, some nausea & bone pain, when I last checked I’m at psa 17at the moment, Happy trails until we meet again
