Hello everyone, I'd like to share that my father experienced an increase in liver enzymes due to abiraterone usage. Doctor made the decision to halt the medication twice, each break lasting for a month. Notably, his liver enzyme levels decreased when he ceased taking the medication, but unfortunately rose again upon resuming it. In our second attempt, we adjusted the dosage to three tablets instead of four. However, even after a month on this adjusted dosage, his liver enzyme levels continued to rise. As advised by the doctor, we are now considering switching to a different medication. I'm interested in learning if there are any alternative approaches available in this situation. Additionally, I'm curious about the number of alternative medications that exist after abiraterone. Thank you for your insights.

Xtandi does not have such an effect on liver. So I think they usually switch to it in such cases.

After Xtandi runs its course, its efficacy can be extended with Docetaxel.

And than in case only bones and lymph nodes are affected you could go to Xofigo. If PSMA expression is sufficient and no discordance with FDG scan, Pluvicto and or Acc-225. All radioligands.

Some also benefit from Provenge (immunotherapy).

In case of BRCA mutation, you can get good results from PARP inhibitors and if tests show high MSI burden, Keytruda can have miraculous results.

This is how much I remember now.

I went from 4 to 3 to 2 pills daily. Have been on that dosage for 5 yrs.

Any chance your Prednisone dosage might be the culprit? I know some have to adjust that to get it right, but not sure about liver impact.

The same thing happened to me. Abiraterone, even in a reduced dosage, made my liver numbers go up. It’s a well known side effect of abiraterone in some patients. The numbers go back to normal when you stop taking it. I changed to Xtandi (enzalutimide) and have been on that drug, along with Lupron, for almost two years. My PSA remains undetectable and scans are clear.
