pelvic lymph node tumour

10 years after Brachytherapy a petscan has indicated a 1cm sq tumour in a lymph node in the pelvic region. (what a bugger.)

I saw the image on the computer screen of a speck of light - it's hard to imagine it was cancer???

the mri scan & the petscan confirmed no cancer in any of the organs.

where do I go from here. How do I know that the spec of light was a cancer?

It has been indicated that a SABRE treatment would be best followed by Hormone treatment. Does anybody have any experience of this

You can get the met removed by SABR or SBRT radiation. Then continue with hormone therapy for six or twelve months and then see how the PSA value develops.

If there is cancer in one pelvic lymph node that you can see on a scan, there is certainly cancer in many more pelvic lymph nodes that you can't see on any scan. So the whole area has to be treated with radiation, not just the one lymph node. It will also require 2-3 years of hormone therapy.

It is a tough diagnosis to absorb but hey ho! At least I have had a good life up to now (age 76) but of course it would be nice to see many more sun rises!!

Would you be able to explain the side effects of the 'sabre" radiation treatment and a couple of years of ADT

Fyi I am in the UK and had Brachytherapy in 2012 and lucky not to have had any symptoms being diagnosed via a routing blood test! Had a Gleeson of 7

SBRT or SABRE is not usually used for a wide area like that, except experimentally. It is usually only for a smaller area, like the prostate or metastases.

The risk and kind of side effects are low in similar with any kind of radiation. If you already had brachytherapy, you are familiar with the potential side effects.

2 years of ADT typically has the side effects of loss of libido, hot flashes, fatigue and changes in mood. Other side effects are more rare.

A heavy exercise regime is the best thing you can do to counter the side effects of both.

Whole pelvis radiation plus 2 years of ADT and abiraterone offers a good control of the cancer.
