My doctor ordered this for me. I am castrate resistant with a climbing PSA even on Elegard and Orgovx. Anyone have insights on it? thank you

It seems to be a good test if they can collect enough circulating tumor DNA.

thank you, you have to be a doctor to translate this into a benefit to somebody with castrate resistant stage four. I have been feeling great then the last 24 hours I hurt everywhere, even my teeth hurt. I have the blood test Tuesday, hoping against hope. I am only 64

They study the mutations which could guide the doctors to use drugs such as olaparib, rucaparib, keytruda or other drugs being used in clinical trials. It may have also a prognostic values depending on the mutations they find.,

I was on Olaparib for close to 2 years, keeping my PSA undetectable, as I was found to be BRCA2+ from genetic testing.

I had blood work this morning for the Guardant liquid biopsy test. Expect results in 7 days, which will tell me what mutations may have changed and what treatment options are available.

Olaparib and rucaparib have been approved for mCRPC with mutations such as BRCA, ATM, PALB and others.

The advantage is that it includes DNA from all your metastases. The disadvantage is that if your metastases aren't shedding cells when they take your blood, it won't show up. It's also very expensive.

The genomics of biopsy tissue is sure to get i ts DNA, but it will only get DNA from the one site. It's not a big problem if the genome isn't changing very rapidly, which is often the case.
