Personally, I am dealing with advanced stage 4 with bone involvement so, with that as a back drop, here is my humble opinion. If nothing is showing up on scans or mri’s I would chose active surveillance. ADT is a heavy hammer for something that may not even be there. PSA is an indicator only. Quality of life baby!

Understand and respect the viewpoint, OGR. However you are already metastatic regionally and to your spine. Monitoring its progression in the absence of systemic treatment, if that is what you choose is not what is normally meant by “active surveillance “ as it is used in early and indolent PC. Good luck.

I don't understand. You said you have "bone involvement" but then write "If nothing is showing up on scans or mri’s..."How do you know you have bone involvement without a scan of some kind?
