Fist time posting, see hubby Hx on profile.

Recent PSMA shows disease progression in lungs and inguinal nodes. PSA remains .003. Still on Lupron/Zytiga/Prednisone. What are next steps in treatment? Overall he is feeling well, exersizes everyday, good appetite…

Continue with Zytiga/Pred. This should stop further progression.

Sorry that I have nothing to suggest but only would like to thank you for the useful info you provided. You see, there are silly docs and their parrots that claim no PSMA detection bellow 0.5 PSA.

I think if you don't have pain than they wait with the PSMA PET scans until your PSA is 0.5. if you have pain than they could order it below PSA 0.5. At PSA 0.1 you have only 30% chance to find something on a PSMA PET scans without pain present.

His MO does annual PSMA scan for continuity/comparison. We are so fortunate that Fundación Valle de Lilli here in Colombia has top rated physicians and technology!

If he is showing progression in his lungs and nodes with PSA that low he may have an unusual type of prostate cancer. Ask for a biopsy if there is enough tissue.

Could he have a liquid biopsy instead of a surgical biopsy if he really ends up with not enough biopsy tissue available for analysis?

I don't know that liquid biopsies are much good when the apparent size of tumors is small. Also, they only provide genomics.

I have the exact same thing. With metastasis being only in my lungs. I was given Prednisone ,Zytiga and Lupron. I did the trial for the PSMA at UCSF. That's how it was found. This was 4 years ago. You can look at my history. I am currently on ADT vacation. Both my oncologists, the one at UCSF and locally are totally for this. I didn't have biopsy. At UCSF they felt scan was the true indicator of it being prostate cancer.

Ask Doctor if Keytruda will work on your lungs......It did for my Lung Melanoma.....

Thank you for sharing your experience. Have you repeated a PSMA scan? To date my husband has had 3 and his most recent scan demonstrated increased pulmonary activity. This has been a big blow for us as he has been feeling great. With this disease you can not let your guard down!
