Abdominal and back pain??

Good day to all, just wanted to know your thoughts please (and please forgive my lousy english)

My father has been suffering from pain on his mid section(mostly mid portion of the back and some on his abdominals). Sleeping has been harder because of this. On its first week he believes it was caused by an awkward sleeping position, perhaps it stretched his mid section. We are not comfortable with pain drugs so we just apply pain relieving gel, it seems to have been gone in a week of applying the pain relievers and being careful with sleeping position until he accidentally gets into a similar position and stretches the muscle again and we get back to zero. My initial fear is it could be prostate metastasis, his last bone scan was february 2021 and it was a lot cleaner than his july 2017 bone scan. To be sure we also got him scanned last friday and the results will arrive on wednesday.

IF this is not metastasis then I could only guess its muscle pain, he is under Zoladex injections every three months, could there be a relation? Our Urologist doesnt see any connection with Zoladex though. Upon consultating with an Internist/Cardiologist, he was just given tramadol + paracetamol for the pain.

Your thoughts and suggestions would be much appreciated.

Sorry your Dad has been struggling with pain and discomfort. I know it’s hard but you will only know for sure when the scan results arrive. Wishing you the best outcomes.

Thank you sir, submitted the scan to the urologist and indeed it was mets, psa was normal though at 1.72 so the uro believes it is not yet castrate resistant, only new medication given was zoldron for the bones, we asked about side effects since we are always afraid of it, the uro believes it depends on the brand as none of his patients get bad effects.

For now we have no choice but to wait since there is a shortage of the bone medication, only pain reliever is tramadol with paracetamol and pain relieving gel. But dad is feeling that pain might get worse before medication arrives, could there be anything else we can do to lessen the pain while the medication is nowhere near? Thank you very much

Can you get Celebrex (Celecoxib)? It works well with Zoldron.

Castration resistance is a gradual process, not an either/or thing. But if he has new and larger metastases while taking Zoladex, the process has started. His pain may be relieved by docetaxel, Ra223, or targeted radiation.

Much appreciated again sir, could you also enlighten me please, is zytiga and xtandi another adt type of treatment too? If so then how exactly does it help if the cancer is already castrate resistant? Thanks much

They work even after the cancer becomes castration resistant, so does Zoladex (it is especially important to prevent any testosterone). Castration resistance just means the cancer progresses in spite of having little testosterone.
