Treat BPH in prep for Brachytherapy?

Hello and thanks for reading.

I have done some study and would like to go the brachytherapy route. But I have a weak urinary stream.

I am thinking that if I go that route, I’d better treat the BPH... beforehand.

Has anyone here done that same thing?

What would be the best approach in terms of lowest risk of spreading the existing PCa?

Do you have advanced prostate cancer? Men on this forum are usually on ADT anyway. In the ASCENDE-RT RCT, everyone got 8 months of ADT (prior to BT or EBRT) to shrink their prostates.

If you are talking about some kind of TURP first, you have to wait a while before radiation.

How big is your prostate? How many metastases do you have?

Hi Tall Allen,I have G(4+3) believed to be organ confined at this time.

So you do not have advanced prostate cancer, and why do you think you have BPH?

Well, my urine stream is pretty weak and the bladder does not empty.

Consult a good experienced Urologist as well as a radiation oncologist to sort it out.

I also had a weak stream, diagnosed with prostate cancer with a Gleason scale of 9. PSMA-PET scan luckily showed no metastasis elsewhere. Had a heart attack and could not have brachy treatment for 6mths. So went on Zolodex then had high dose radiation with Brachy followed by 23 sessions of external beam radiation. My PSA is now .008 and I have to take Duobluts to help me pee. My Urologist is confident I may have this beat, but I will need to take Duobluts for my remaining days. Go for Brachy followed up by EBRT. Good Luck
