I have metastatic disease on ribs and pelvic bone. It’s been over 4 years on single ADT. Scans show no new metastatic spots and PSA at 0.2 and ALP at 94. Recently the last year I have noticed an increase in pain and discomfort in my back and ribs. Mostly notable when lying in bed.
I have always been active and do physical work and exercises. I had a bone density test a year ago and it was good. Am I missing something. I am trying to carry on with life as I did before treatment. Should I not be as active due to the Mets? Is the pain caused by my bones becoming brittle or just inflammation. I know I am living on borrowed time, just trying to make it through.

It's hard to guess what is causing the pain. My understanding is that bone pain is from micro fractures. My pain always seems to be in the muscles and nerves, so I think of it being inflammation.
Whenever I try to find the cause for a problem, I try to find what input changed at the time the problem showed up. Your PSA values are consistently low. What else may have changed in your activities, or health?
Twice now, I've had spinal lesions pressing on nerves. My MO ordered MRIs both which found the problems. You might ask your doctor about an MRI of the area.
I had scans done last month, CT and MRI both with contrast. Apparently nothing new noticed.Nothing has changed with my activities.
I am noticing a great deal more aches and pains in my core. I guess it could be the loss of strength and I am straining what muscle I have left. Definitely a big change in the way I feel compared to a year ago.
Have you had MRI or CT scans recently? Back pain can have many causes. Even if it is caused by increased mets it can be treated. But you need to know more first.
As your father and grandfather had PC it would be a good idea to have genetic testing done to identify any relevant mutations e.g. BRAC1 and BRAC2. There are treatments that can be useful if you have these e.g. Olaparib.
