Hello Seattle Dan....Do not want to sway you one way or another..Just wanted to share with you because I had to make a similar decision...The areas medistinal and superclavicular are areas where I they have found metastasis on me. Was on 1st line ADH when 6 spots showed up on a PSMA. Previously had one spot which was radiated but did not result in log term durability. Was ready to radiate the six spots but was delayed because of covid. During that time added xtandi and when it came time to do it it was hard to pin point all the areas. I think one of the 6 was not in a area most ideal to radiate. Right now am still on Xtandi. Have not had any scans since but psa is down. Always planning ahead to next step but trying to treat as chronic disease with new treatments out there. I have stepped out of SOC and would always consider clinical trials . Want to do what I can while I feel I can tolerate treatments. With radiation, my thought even though unproven was that I might get some obscopal affect. Good luck in your decision Dan.. You have to believe that what ever way you decide that it is the right decision and do not look back . Your heart knows. Much grace.

Be very careful about radiation in the mediastinum - even small doses to organs up there can be dangerous.
