I think it’s a tough call for what to do with mCRPC. Many have noted that Lu- PSMA isn’t a cure, and on average adds less than a year of survival. Unfortunately, other treatments are no better.

It seems we try a mix of treatments hoping that we become part of the group with the extremely good response on one of the treatments we try. I sometimes feel like a test pilot in a malfunctioning plane. I’m going to try every corrective maneuver I can, until I try the right one.

The funny thing is that in my first profession I am a military pilot ..)) I was 19 years old when I first took to the sky.. There were a lot of devices in the cabin, but I knew the purpose of each, I knew everything about my car to the very last screw and I knew why and how it all works.. All that remained was to control this complex machine with two hands and two legs, and I was calm! The human body, as it turned out, is much more complicated than the most modern computer and we don't know even a 10th of what we should have known about it.. Today's medicine treats us as average patients, without taking into account the individual characteristics of our body, without taking into account concomitant diseases, without taking into account the fact that a tablet for diabetes can enhance or weaken the effect of a drug from oncology and much more.. And unfortunately, in such conditions, we have to reach everything with our brains in order to find the most cherished key to our healing.. As sad as it may be - but that's exactly how it is! For six years of my journey, I have passed all the standard treatment regimens that are available in Russia.. My own choice of experimental therapy 177Lu + 225Ac - PSMA outside of Russia was a step of desperation, the last line of defense.. And only now, after I have already passed 4 courses of such therapy, I realized that I was just lucky that I met a gram-literate radiologist, that my foci do not accumulate FDG, that I was taught that with any therapy it is necessary to keep my T at zero.. And recently I found out that maybe my aggressive cancer and a possible breakdown in the BRCA genes contributed to a good response to treatment with PSMA.. But I didn't know all this yet when I agreed to such treatment! It just all came together at the right point at the right time by someone's will from above!

Where did you have your PSMA treatments?

To the Nuclear Medicine Center in Baku, Azerbaijan. In terms of price-quality ratio, this is the best option I have found.. Plus, this center is an active member of the European Association of Nuclear Medicine, and this in turn is a guarantee of quality and safety! Many people have already asked me to make a separate post about this center in Baku and tell about my path to this treatment.. I'll probably decide and write about it so that those following me don't repeat my mistakes.. It's worth writing about it for the sake of it!

Thank you for being part of our group. Keep posting!!!

Before I leave here's one for you. How does a Russian pilot navigate his plane? By reading the street signs.

Good Luck, Good Health and Good Humor.

Hello, John! If necessary, a Russian pilot can sit on the highway near your house and treat you to a beer.. We call it the restoration of spatial orientation by communicating with local residents..))) A good old joke from the very days when fighters were still with propellers!

Dear, j-o-h-n! I already realized that you are a wit in this community and it looks like you have Russian roots.. I also like to joke, but I never go beyond what is allowed and never offend human feelings with my jokes, which are prohibited by the rules of any civilizational community! I will answer your inappropriate joke in Russian: if circumstances require it, a Russian pilot will wind up on his propeller anyone who will not move in the direction he needs.. Like my grandfather did when he flew a fighter with a propeller during World War II.. He died in December 1942 in an air battle with the Nazis near Leningrad.. Please draw conclusions about this and don't try to hurt my feelings anymore! This is a request! Thanks!

Russians have feelings? Anyway, a song to soothe them:

Growing up in NYC we learned this Russian song -

Pizzonya Pizzonya Pizzonya, in Russian that means I love you.

If I had my way, I'd Pizzonya all day, Pizzonya Pizzonya Pizzonya.

(Greek - Orthodox roots with Humor for blood).

Good Luck, Good Health and Good Humor.

Well, then you should know better than anyone here that Russians have a broad soul and a big heart, in which a lot of feelings live.. But the Russians still have brains that do not allow these feelings to break out of the bounds of reason ..))
