Possible uptake after pet/scan but PSA is still 0.01?.

Hi Folks, I had a petscan in late August and 2 weeks later one of the Oncology team rang to say they were not sure but it looks like there maybe some uptake in the L2 where I had high dose radiotherapy last year & now have a spinal fracture of L2.

Plus also possibly a spot in the Hip.

I just had a PSA test which came back as 0.01

So does that mean there is no cancer uptake in those areas, I would think my PSA would go up before it was detectable by a scan yes/no maybe.

Any thoughts

The scan is what matters. Bone alkaline phosphatase is sometimes a useful blood test.

If the radiologists are not sure of the diagnosis, ask about having a MRI of these areas to see if there are changes in the bone marrow which could be consistent with metastases.

Hopefully the MRI comes back with positive news. Did you feel anything different leading up to the L2 fracture or at the hip to indicate a met? BTW: thanks for posting this because it brings awareness to a lot of us at at low PSA who are basically going test-to-test wondering when the next shoe will drop.

I didn't feel any different leading up to the L2 fracture it just hasn't healed very well so I have a Op next week to put a plate & screws in to hopefully give me some pain free mobility. My hip area I get the occasional pain but I initially thought It maybe due to lying down so much.

I have spent a lot of the last 6 months not been able to exercise much & lying down a lot so I have been losing muscle mass and getting pain in my upper legs so hoping I can start to exercise & walk a lot more after this.

Thanks for relaying your experience. It's hard to know which pains to listen to and which ones to ignore. I have a persistent rib pain at 0.02, and don't know whether to have it checked or not. My Dexa was -2.1, near osteoporosis, so I'm more apt to be wary of a pain in the vertebrae that might lead to a fracture. Cheers- Tim

No problem m8, I felt a little bit stumped so I always come here the guys are a great help.My L2 there was no pain leading upto the fracture as I had radiotherapy on that spot a few months before which was partly to blame for the fracture.

Just want clearly the role the radiation had on the L2 fracture. So the radiation killed the tumor material in L2 and that caused the new fracture in the L2 bone material or was it some other process that caused L2 to fracture , say damaging the osteoblasts to be damaged leading to bone erosion or …?

A couple of months after having the high dose radiotherapy I was trying to lift something a bit 2 heavy when I felt the twing, the radiotherapy contributed to the fracture, But wasn't necessarily the whole cause.

Some uptake is possible in areas which had past injuries on bone..But the orange shadows will only be lighter if it is not PSMA avid bone met.The intensity of a lesion (how bright it shines) is measured by SUV Max. A SUV Max of a lesion is 10 or less, it most likely is not a true met. Higher the SUV Max of a spot..higher the chances of it being a true met.

Doesn’t it depend in what the liver SUVmax is? As in, if higher a true met?

Can't recall sorry was 2 months ago.I just know they inject some stuff into you then do a scan twice.

The most common one is Gallium 68 PSMA PET CT. Ga68 is injected in your forearm. Then you are made to drink a liter of plain water for Gallium to distribute in your body uniformly. Then scans are taken . The whole process takes about 2 hours.

MRI did not show any cancer progression woohoo, it did show some sight wear and tear on my hips though due to the hormone treatment, I generally take calcium take tablets to try and combat this.
