Second biopsy 3 years later shows GL 10 and neuroendocrine

Second biopsy showed progression of disease in my prostate as PSA is doubling every 30 days even after 7 courses of docetaxel. Biopsy results here for any tips or things I need to make sure to discuss with my MO. Thanks!

Gleason 5+5 all 12 cores

Core Neuroendocrine: 5%, 15%, 10%, 20%, 5%, 20%, 5%, 80%, 70%, 90%, 10%, 20%, 70%

IHC: Result

Pan-cytokeratin: Positive, with perinuclear dot-like pattern in small cell carcinoma

tumor cells.

PSA: Positive, expression is reduced in small cell carcinoma.

INSM-1: Focal positive

Chromogranin A: Focal positive

Synaptophysin: Focal positive

At Duke, they can stain for DLL3, which may qualify you for those trials
