Prostate stage 4

ER Doctor is suspecting UTI,,,The first thing is to get Urinalysis and Urine Culture and sensitivity to know exact name of bacteria and the most effective antibiotic to kill it. 80% of UTIs in prostate cancer people are caused by E Coli. Remaining 20% are caused by Klebsiella, Proteus, Chlamydia, Gonococci and others.
While waiting for culture results, the doctor needs to put him on antibiotics such as Ciproflox or Doxycycline ..and give Craneberry capsule with D Mannose. A swift action
against UTI will save him a lot of trouble. Due to impaired immune system, UTI in PCa people can quickly spread to testicles, scrotum and other pelvic organs. Warm SITZ baths can give comfort and reduce inflammation in the area.
It is common to get a UTI from the catheter. Also, if the stent gets blocked up and urine backs up to the kidneys, it can cause edema (the ankle swelling). The doctor will have to do some diagnostic analysis.
Discuss having an urinalysis with reflex culture and a metabolic panel to determine if creatinine is elevated. If he does not have venous insufficiency, swelling of the ankles along with swelling of the scrotum and penis may indicated he is having edema. There are many causes of edema,. With his clinical condition, the edema most probable is caused by the kidneys problems.
My Dad had pelvis, scrotum, lower abdominal, leg and ankle swelling due to lymph node metastasis. His lymphatic system was damaged and he unfortunately still deals with this issue 4 years later, although he is still alive! He also has bladder damage and uses a catheter now, which caused a lot of swelling as well, Hopefully that is not the situation for your Dad and it is just an infection. Best wishes!
