Looking for anyone or anyone that knows someone with a post RP low but measurable uPSA level? like to know if anyone has avoided BCR.

Looking to engage with anyone or anyone that knows someone that has been or is in my situation

I am 4 years post RP with two years of rising uPSA from <0.006 to 0.012

Many say I have nothing to be concerned about so I would like to find ONE guy that has travelled the path before me,

The production of the PSA enzyme by other organs can register measurable amounts in low concentrations. The PSA molecule is very similar to kallikrein, which is androgen-dependent and produced in epithelial and glandular tissue.

After over 120 PSA tests, I have never had a uPSA. My research medical oncologist told me that he would never use that particular test in that it would cause panic at each rise - ever so minute. Ask your medical oncologist on why he wants this particular test as, unless in research, most do not really know how to interpret the results.

You are undetectable with a <0.1. If you continue with the uPSA, just understand that other tissues in your body can and will express minute quantities of PSA enzymes in your vascular system from sources unrelated to prostate cancer.

You might want to enter into a discuss with your guy on this topic. Good luck. Stop worrying. There is no need to panic with a minute rise and under certain circumstances there could be a minute drop as well.

if the uPSA test is unreliable I hope to find someone else that has a uPSA history that does not trend upwards

many think that once the uPSA crosses 0.03 you are past the point of no return in terms of BCR

It’s not that a uPSA is unreliable. It’s a great tool, however, what good is a tool if one does not understand its use. For example, your post is indicative of the problem - lack of knowledge and understanding. In most cases, the number that you should be aware of is undetectable or <0.1. Doubling time models do not take in account numbers in the thousandths. A rise of .006 to . 012; especially with non-prostatic tissue contributing to the universe of PSA enzymes. I am wrong, someone please correct me. My view is based on my time going back 16 years forward. As I am not an expert in the matter, it was suffice for me to not suffer from worry with the results of uPSA.

Now writing on BCR brings up an interesting point. And that is the concept of micro-metastases; ie the unseen cancer cells floating in one’s vascular or lymphatic system looking for a place to land and colonize growing into a metastatic lesion. Another worthy topic to learn about.

I wonder if anyone knows why or has experienced a rise on PSA level (from 70 to 274), 6 months after radiation therapy (VMAT x 26 sessions)?

Before and after radiation therapy, dad was taking Bicalutamide 50 mg and Alfuzosin50 mg together...

Does a pathologist know if adenocarcinoma is neuroendocrine by looking at the initial biopsy or is this something that is discovered later?
