What do people know about bone treatments for bone mets? Currently we are using targeted exercise as our bone mets are small (albeit numerous) as I figure the side effects of targeted exercise are better than side effects of drugs...

What do you mean by “targeted exercise”?
If you are talking about bone strengthening agents to prevent fractures, he should have a DEXA scan to see if he needs that. Yes, weight bearing exercise may be enough to keep bone mineral density normal, but if he has taken ADT for a while, he should have a DEXA scan.
I take xgeva shots quarterly since having bone mets and stopped ADT which can cause osteoporosis and switched to estradiol patches. Also do weight bearing exercise daily. My bone density is normal. Test it annually. Plus I lost 50 lbs to take excess weight off my skeletal system .
I had radiation for bone mets on my hip area and I was also on Eligard injections. So far everything is OK and I see my Oncologist on the 20th.
I had LOTS of bone mets at original diagnosis and was prescribed periodic IVs of Zometa (zoledronic acid). Personally, I've had no side effects from it, other than the usual hassles of blood work and IV sessions. I've known other men with many bone mets who have taken alternative Xgeva (denosumab) injections, with similarly few side effects.
