How to know when Jevtana is no longer working, is it PSA alone?

My husband's PSA has bounced around while on jevtana - 8 cycles now - but just spike 50% over a 2 week period. Have a call out to his onco but would love feedback here. Thank you!

You have to look at the whole picture, especially scan results. PSA can become a less useful biomarker over time.

My husband's PSA has never been higher than 18 in an 18 month period, but his scans from initial diagnosis have shown extensive bone mets. His oncologist told us that we should use only the scans to determine the effectiveness -- or lack thereof - of treatments.

I agree, you have to look at the bigger picture. You don't change treatments based on PSA alone. Imaging is more important along with how the person feels, pain symptoms, etc.

Take a PSMA PET-CT scan and see whether the tumors have increased in size and qty. If they have, stop Jevtana as it is obviously not working. Only screwing up your husband's immune system.
