Hero Dad diagnosed with APC with PSA > 100, Gleason Score 4+5 = 9 and multiple metastases in bones and Lymph nodes. Need guidance from Heros

Hello All,

My name is Shahbaz. My hero father was diagnosed with Advance Prostate Cancer (APC) a week ago. His PSA number is more than 100 (unfortunately I do not have the right number), Gleason Score of 4+5 and metastases in numerous lymph nodes and at least 6 bones.

Apologies, if you find my questions naive. I am new to this situation.

Is my father's condition already resistant to one of the Lupton-like drugs? If yes, what are the options we have at the moment? Considering all the medical advancement in mind.

Personally I dont feel qualified to make specific recommendation but the orchiectomy should be effective in slowing the growth of the cancer and have the same effect as ADT (lupron) without the side effects, which may or may not be significant, depending on the individual. If your summary is comprehensive I should not think his cancer is resistant to ADT (lupron).

The way you tell if he is castration-resistant is if his PSA continues to rise while taking Lupron or after orchiectomy. Considering the high volume of metastases, docetaxel now is probably something he should consider. Abiraterone is also good in this situation. Here are some options if he is still hormone sensitive:

Thank you very much Tall_Allen for your quick reply.

Tomorrow he is going to have orchiectomy that means we will have to wait for few weeks in order to establish if he is castration-resistant or not.

Thank you again for your guidance and recommendation of Docetaxela and Abiraterone. We will consider them and also other options once we establish his condition and response after orchietomy.

2008 was dx with 4 stage prostrate with mets at the time just in my lymph nodes. as time moved on some mets in my pelvic area aortic arch,rib and now the back. gleason score was 7 at the time. its now going on my 11 year and other then some problems with side-effects of the treatments nothing else. i'm 72 now was dx at 61 and i feel good. so just get u a good trusty oncologist, or urologist. if not happy with the treatments get a 2nd opinion its your life dude

Father seems to be satisfied at the moment with the oncologist we are consulting and I guess that is the most important thing. Your story gives us strength and motivation. Kindly continue writing to us.

Here in USA, Orchiectomys are rare... I've had one and wouldn't go back if I could. Then chemo + Xtandi. All working for me. Good luck.

I do not know how rare Orchiectomy is in India, but my father is going for it. Reading your story gives us strength and confidence. I hope it will work for him too.

Thank you for your response and apologies for my late acknowledgment. I was busy with the line of treatment and care of my father.

I will certainly keep in mind to give complete details of my father. Just for you, my father is 60 years young, He is based in India and currently getting his treatment in Medanta the Medcity hospital in Gurugram (Near New Delhi).
