How Prostate cancer become neuroendocrine (NEPC), resistance to treatments and deadly

Some patients receive anti-androgen therapies may develop a deadly treatment-resistant prostate cancer subtype called neuroendocrine (NEPC).

The cancer cells are completely reprogram themselves . Similar to a cold virus mutating to become resistance to a particular drug. The tumor even loses the receptor that is targeted by current treatments.

These targeted therapies are the cause of the PC becomes more aggressive and deadly.

This article describes the transformation of PC to NEPC type, also the treatment to prevent this transformation.

Could this occur with the use of the antiandrogen bicalutamide (Casodex) used as monotherapy?

Yes. Casodex is the first drug used after you become castration resistance. Is called the first line of defense. You still have many drugs ahead of you. 3-4 years , by then there will be a lot more drugs. Enjoy your time.
