What are some levels you have had with Prostate going to bones in a very short trip me.

I don't know who long it took to go to my bones, but my Alkaline Phosphatase was something like 605 at diagnosis. That was with extensive mets, a lot of pain and a PSA of 463.

My was 180 it's now down to normal range I think its like 80 alkaline phosphate I'm guessing its the chemo that I'm on

Mine 18o down to normal of 80 after 3 chemotherapy treatments

One of the things that I learned is that there are two Alkaline Phosphate assays.

1. is regular Alkaline Phosphatase (34-104)

2. Bone Specific Alkaline Phosphatase (7.6-14.9)

Tall_Allen or POshea.... do you have any comments on the difference between these two?

Not sure what I was at diagnosis, but I was 451 on 10/31/18, spiked to 905 by 11/14, and am now down to 80 as of yesterday, 1/22/19. The spike may have been caused by starting Eligard alone without anything to limit the initial spike in T, or by radiation therapy, or maybe even a side effect of a TURBT procedure. Suffice to say I had a lot going on in early November.

My ALP is being tested weekly while on docetaxel. It seems the week after each infusion there's a bigger drop in ALP, than other weeks.

cesanon , I was just reading about ALP, and it seem that differentiating between total and bone specific ALP is only meaningful when values are near normal. If total values are very high and there's evidence of bone metastasis, you can assume the ALP is primarily of bone origin. They even said the cancer cells can learn to produce ALP themselves after hanging out with bone cells, those f***ers!

Hello, I had prostate cancer in the prostate gland only in 2007, then in 2011 my oncologist noticed an increase in my alkaline phosphatase test. He order an MRI and found metastases on my hip bones. I had an additional 30 radiations and started Casodex for 3 weeks with Lupron injections (Eligard - generic) for 6 1/2 years. At this time I no longer have prostate cancer. My PSA is 0.006 at this time and it has been 9 months since my last injection. Ask at least two doctors about what you should do next and above all just keep truckin'.

No, I am a 73 year old gay man who enjoys going to a gay church and have many friends in my life. I am a member of Narcotics Anonymous (for 35 years) and sponsor many people. I am also a retired Pharmacist and worked or CVS for many years in Ohio. I also worked for a suicide prevention hotline in Ohio, I now live in Orlando, FL.

So I highly recommend that you just keep truckin'.

I went to a shrink who told me to lay on the couch. Then he asked me "what I did for a living?" I told him "I was an automobile mechanic". He said "then in that case get under the couch".

My Alka Phos at dx was over 500, I had multiple mets throughout my skeleton and pain. After triple ADT my Alka Phos has been staying in the low normal range between 47 - 56. Bone specific AP has also been in the low normal range and has remained there for quite some time. This coincides with scans showing no active disease.

My local medonc usually tests for both since AP is also produced by the liver so bone specific will only show if there is an issue going on with bone. However usually both would be high since the bone related AP would also show up in the regular AP as an elevated amount. AP is one of the key tests to have done in determining disease activity especially if you are on long term ADT with drugs like Xtandi and Zytiga. It may detect the presence of active non-PSA cancer, which is becoming more common with the longer survival time of guys on Xtandi and Zytiga.

My Alkaline phosphate level at first diagnose was 3844, 10/24/2017.

My ALP after doxetel, Zytiga, and Xgeva and Lupron fell too the 400 range from 650.

Off Zytiga and on Xofigo and labs coming soon.

Hoping to trend downward if I can make it through Xofigo which has not been easy.

In late 2013 my Alk Phos was 424 with a PSA of 5,006 at original diagnosis.

Alk Phos dropped to the 45-55 range while initial Lupron (& Zometa) had PSA below 2.0.

With castrate resistance, Alk Phos rose into the low 80s as PSA rose as high as 95.0.

Started Xtandi in Dec 2016 with Alk Phos around 80. Current PSA is 3.1 with Alk Phos 28.
