HIFU is a local treatment. I'm not aware of its ever being used for metastatic disease. If you have local disease, including recurrent disease after failed surgery or radiation, HIFU can be tried.
It is my understanding that in the early years of HIFU treatment the results were not as good as for surgery or radiation. Some say it has improved since then. I don't know. You can search the Internet (especially Pubmed) for studies comparing outcomes of HIFU and other treatments.
In my personal opinion, I think it can be a mistake to pick a treatment and then go look for a doctor to do it. So much depends on the doctor. Is he careful? Is he truly committed to his patients? Is he experienced? Does he listen when you talk, think about your questions, and answer them usefully? Does he tell the truth or just boast about how good he is and say you'd be crazy to go to anyone else? Don't go for HIFU if you have any questions about the available doctors. Good surgeons and radiation oncologists, or good medical oncologists if you have metastatic disease, are available.

You nailed it Alan. I am sticking with my highly recommended RARP surgical onc. 12/1 is the date. My fervent prayer is he is able to reach an involved posterior iliac LN precariously close to the rectum. Likely not. Many thanks. This site is a God send.
