I had written here that I was going on the vt-464 trial. Well that won't happen as the trial is closed. Has anyone had any luck with Xtandi after Zytiga?

Xtandi after Zytiga
I was on Xtandi after Zytiga for about 4 months. It didn't work. I am now having Chemotherapy using taxotere every 3 weeks.
Zytiga two and a half years ,went to 1,000psa. Four months on xtandi, now down to 5.00
Funny you post this today, as my MO and I had a very lengthy discussion on this topic yesterday. I have done 6 rounds of Docetaxel, and been on ADT for over 18 months. This past summer when my PSA was doubling every 30 days, the MO put me on Zytiga and Pred...first few weeks I felt better than i had in months and PS dropped from 225 to 150 in 4 weeks...then doubled back up, and then again. So yesterday we decided to give Xtandi a shot, and he is having me wean from the Pred for a few week, take a week off from the AA and start Xtandi next Monday...we are giving it exactly 3 months, and if by the first of February we don't see a steady and sustained drop then back on the chemo, long term, to get the PSA back down to my Nadir of 1.45 if possible. I will update as the Xtandi starts next week.
I had the same type of response with Zytiga; big drops in the beginning then worrisome doubling time which made my doc discontinue treatment. And funny again like you, I had a discussion today with him about Xtandi but he's not too enthusiastic about it, for me anyway. I'm really curious how you will respond. Let us know how it goes. I wonder what the issue is with the Xtandi? My MedOnc wanted X, before Z, but my insurer balked. Now that Z it beginning to fail, it's certain, I will go to X. As long as my numbers are ok. And, please post your doubling times and PSA's if you would. Post them privately, if you'd like. I'd like to compare mine, which are not doubling, but creeping up slowly enough to where, if you take one month and times it by 2, it's more than twice as much. If I got that across right.

Ray had 12 months on zytiga, then 12 doses fortnightly of docetaxel and then onto Xtandi. He has now been on this for 2.5 years, his PSA has increased to 26.03, but has held there for 3 months now. Good luck
I got about 1 1/2 years with Zytiga, then Dr. Snuffy Myers tried high dose Ketoconazole which worked for 1 year, and finally Xtandi for about 1 more year with limited effect.
From everything I have read, you will get the best improvement with the first drug, be it Zytiga or Xtandi, and limited impact from whichever is second.
I have always wondered why not use both together to limit testosterone production and to block receptors to the maximum?

Had good results with Zytiga for about 18 months; mild relapse, with new lymph node activity, and increase in PSA from 2 to a little over 3 in Dec 13. I've been on Xtandi ever since (almost four years), with an undetectable PSA and no new met activity. As soon as my PSA resolved after switching to Xtandi, I also did Provenge, which might be part of the equation. I also take Metformin and a statin, and continue with Lupron shots.
