I'm a Kaiser Senior Advantage Medicare member, & they offer Provenge. I believe copays will be dictated by the plan I'm on as a retired California state employee.

I did it a couple of years ago. Traditional Medicare A&B with a Blue Cross Blue Shield Plan F medical supplement policy covered the cost entirely.
God knows what is going to happen in the US. It's tragic what your government are doing to healthcare. Here in the U.K. Everything from cradle to grave is free to all, no exclusions, no excuses. Every employee and employer pays a comparatively small tax based on earnings called National Insurance, which also pays unemployment benefit and state pension. The more you earn, the more you pay to support your fellow man, woman and child. Prescriptions carry a small charge of about $9, but that covers any drug, cheap or expensive. I've been taking Zoladex, Abiraterone, Prednisone and Lanzaprozole along with cocodamol for 5.5 years now, all free. If you get Cancer, or reach retirement age, you get free prescriptions.
Why can't you do that in the land of the free?
We can't, because it makes too much sense. If we did do something along those lines, the CEO's of corporate America won't be able to "earn" the millions that they work really, really hard for. And how dare you suggest that big pharma do what's right, and not gouge us for life saving meds. I live in a big pharma state, where the sh*ts made, and we pay more than Canadians do. Go figure? Democracy in inaction.

Yes covered. I have supplemental plan F, so not sure if there was a co-pay after medicare.
I believe that the benefit is nearly a year on average (if you look at arm-b, corssovers vs non-crossovers), but some doctors feel (Dr Kwon) that the immune system is already on alert if other treatments create cell fragments. So he is not enthusiastic about it. I suspect if it cost less, he would change his mind somewhat.
