Tips on tracking medication change history

Does anyone have any recommended apps or ideas on how to keep track of your kid's medication change history? I am in the process of changing providers for my son and after spending hours digging through MyChart and the pharmacy website it hasn't been easy to piece it all together (4 years of changes and still trying to find the right medicine!) Any tips on how to maintain this information going forward?

I just put a spreadsheet together to document everything. A row for each week and columns for the medication, dose, side effects, behavior trends, etc.. My son has a behavior plan at school so I put that weekly data in a column as well. it has worked out well and my son’s psychiatrist loves it. Once you have figured out a steady medication/dose, you probably don’t need a row for each week, maybe each month. But when making changes, weekly is helpful.

This isn't very sophisticated, but whenever we change my son's medication, I make an event on my Google Calendar documenting the start date of the new medication and dose.

I use the calendar on my phone for everything so I put it in there to keep track of the dates.
