PSA spikes while in AS

Last biopsy in Aug. 2022 showed 2 cores Gleason score 3+4 (pattern 4 ≤20%) from a total of 25 cores. The Artera.AI was in concordance with the Decipher score at 0.3. There were no cribriform patterns or intraductal carcinoma, and no genetic mutations per the PROMISE Study. PSMA/PET scan in May 2023 indicated no metastasis. Tumor is on TZ. MRI in Sep. 2024 shows no change. PSA has been stable for about 5 years at 8.5 with large 70cc prostate. Having been in Active Surveillance since 2019. In Dec 2024 PSA jumps to 11.8. A repeat PSA in Jan 2025 another jump to 14.5. Will meet urologist in early Feb to request PSMA/PET and a Transperineal Biopsy is scheduled in Mar 25. Has anyone experienced big PSA spikes and it returned to baseline due to prostatitis or other causes? How long did it take to return to baseline and whether antibiotics were needed?

Hello, I have been on active surveillance for almost 4 years and am now considered favorable intermediate due to PSA over 10, My PSA has gone up as high as 21 and as low as 11 just in the last year. If you look at my history, I have had 5 MRIs last one Pirads 3 and 2 biopsies with the latest biopsy showing 1 core 3+3 and 1 core atypical. I ride my tri bike about 4500 miles a year and I do other vigorous training as well. I don't know if this has anything to do with it but my PSA was 21 last July and it was 17 on the 22nd of this month after taking several supplements for a few months, cranberry powder, white button mushroom powder, turkey tail mushroom powder and tomato sauce (only two weeks before the test for this one). I will be getting another MRI and PSA in March to see if it goes any lower. Who knows. Oh, I pray a lot too.

I was on AS after a biopsy revealed a few G6 cancer cells. About four years later my PSA went up about three full points and I had an MRI. The MRI revealed a PIRADs-3 area. A follow-up targeted biopsy revealed some G7 (3+4) cells, so I decided on treatment at that point.

I could have stayed on AS for a little while longer, but I figured with G7 appearing, I had maybe another year or two on AS, tops.
