In transition from one state to another

Please someone help me with this. I'm in transition from moving out of one state to another state. How do I find a doctor and how do get my medicines switched over? Right now my sons are supposed to be doing a one time mailing me my meds and I only have a limited time to get set up. Because Doctor says once I fine a doctor I have to let him know so I guest they can collaborate. Another problem I have is family is not reliable and so one of my sons picked up my pro-graph from the pharmacy and did not mail it to me. So the other son is supposed mail it to me and he's not answering his phone I have today worth of meds left. What should I do? I'm in panic mode I have no money to out right pay for medicine.
