What are you hoping for from the result of this blood test?
What is being tested - just TSH, or TSH plus FT4 or TSH plus FT4 and FT3?
If you want the highest possible TSH (to avoid a dose reduction or if you are looking for a dose increase) then you need a test no later than 9am.
Your 4pm blood draw will give the lowest TSH of the day, it's highest early morning and lowers throughout the day.
If you take your Levo at 12 noon and have blood drawn at 4pm then you will get a false high FT4.
To get a result showing the normal level of circulating hormone (FT4) you take your last dose of Levo 24 hours before the test.
If the TSH result isn't important, and you keep your 4pm appointment, then take your last dose of Levo at 4pm the day before the test, then take that day's Levo after the blood draw.
You should not have a thyroid blood test at 4pm. I would cancel it and book a morning test as an afternoon test will show a low TSH and it will be very hard to leave 24 hours between last levo dose and test.
