FULL DISCLOSURE, LONG POST Had a TRUS biopsy end of September after AS of a rising

. Had a 3T MRI in November 2021 which showed a PIRADS 3 left base and PIRADS 2 right mid, prostate volume 40mL. 4K in August of this year was 57.1 with
PSA
of 7.8. DICIPHER from biopsy was .84.

what to do
My
PSA
had come down to 10 but after the last dose of Pluvicto it’s back up to 17. I live on 3 oxycodone per day and the pain reduction is minimal. I’m starting to see what the end looks like. Any suggestions from you guys? Thank you!!!!

in March of this year was 8.2 and then 10.1 four months later, so rising quickly. Biopsy was done at end of August, which showed significant tumor development on the right side and a Gleason score of 4,5 or 5,4 in half of the cores taken.

Survival outcomes and prognostic factors for first-line abiraterone acetate or enzalutamide in patients with metastatic CRPC
Patients with mCRPC who received first-line ARATs had better survival if they had a

nadir≦2 ng/mL or a TTN≧7 months. Further study is needed to determine if an early switch in therapy for those in whom neither is achieved may impact OS.

radiation whack-a-mole?
Now after two years of undetectable
PSA
, my numbers are rising. A recent prostate-specific PET scan shows a new hotspot in C3, the cervical vertebrae, and they are proposing another round of radiation. I got two years from the last round of radiation, though with significant side effects.

I have annual urologist exams; my

2 years ago was 0.6. Last year was 0.5 (admittedly I did not know you should refrain from orgasm 2 days before blood draw). Urologist said 0.5 is fantastic, no need for digital exam anymore.

Pluvicto Aftereffects SMART syndrome?
He had a hiccup with the supply issues, but

dropped significantly and his side effects during treatment were perfectly manageable, until .... We were heading to NM via West Texas when he began complaining of bright light and headaches. Then he got suddenly much weaker.

My Dad Stage 4 Pca - MO happy to settle for PSA 30, as its not 'curative'!

Update on my father
He remains undetectable on the
psa
scale as of now (<0.02) Diagnosis in 2019 . I’m happy for him and this post is just to celebrate that he is doing well. He turned 70 in April and I serenaded him with Mariachi and he loved every second of that.

My weight I was 138kg now I am 107kg Anti depressants certainly My T score is now 24 nmols, my

PSA Rising and Erleada Dose Raised
Over time he has also had breaks in treatment, but after 6 months when

started to rise, he went back on treatment, which took his

decreased from 0.72 to 0.68 but I understand that this is not as important anymore as the cancer is now small cell. Scans are in the next 2 weeks so we will know then if chemo is working or not. Any advice would be greatly appreciated.

PSA drops quickly
Have not done anything but watch my
psa
slowly rise for the last 14 years. Then 3 months ago
psa
got to 5.5 and a psma pet scan showed cancer on my hip. 30 days ago they zapped my hip with 5 rounds of radiation and I started Erleada and 2 firmagone shots.

Update On Pluvicto Treatments
After the that Feb 13th session I was tested on March 23rd, my
PSA
was down to 244.80 After the second session that was given on March 27th, I was tested again for
PSA
again on May 4th.

is down to 160.40 So things seem to be going in the right direction.

So after radiation and 2 yrs of ADT/Zytiga where my

was undetectable - i went on a vacation. Then my

started to come back - I had an initial PETscan in October 2022 which showed nothing and waited for the

Extra Mammary Paget's Disease.
PSA
rising since, now 6. Due to Ulcerative Colitis, unable to have radiotherapy, so now it's incurable. In March 2023, diagnosed with this new cancer. The frequent leaking was assumed to be the cause of the rash around the scrotum.
